First, let us have a look at what would happen to the blood in the legs if circulation was decreased.

When blood circulation has been impaired or reduced, what happens is that the blood starts pooling in the veins. This does not necessarily occur in the arteries as the blood here is pumping out the heart and isn’t as affected by gravity. But, the blood in the veins has to try and work against gravity to make its way back up to the heart.

When the circulatory system has been impaired then this blood pool, especially in the legs, which then creates a hospitable environment for ulcers. It is more likely for these to develop on one’s legs, but it is possible for them to appear in other parts of the body. Another reason why a person with bad circulation is prone to ulcers is that of how bad circulation affects a person immune system.

If blood is not flowing correctly, then a person’s immune system is compromised. When this happens, then it will take longer for them to heal which is another thing that will contribute to the development of ulcers. This symptom of poor circulation is one of the more visible ones as an ulcer on the legs or feet of a person is incredibly hard to miss. There are also not many other conditions which will result in this kind of skin condition.

15. Inability to perform strenuous tasks

The secret to living a healthy and happy life is no real secret. There is no mystery here, and there is no real debate. There may be people out there who would want to argue the finer points, points like whether it is healthier to be vegetarian or whether humans need meat to survive. These points will be discussed until the end of the earth. But at the core of each argument is that to be healthy a person needs to do a few simple things.

They need to eat healthy food that is not overly processed, and that is not loaded with chemicals. Added to this diet is exercise. A person who is not exercising is a person who is certainly not healthy. Now, this is all well and good, but what about the people who physically cannot exercise.

Put aside fitness levels for a moment and think about what it must be like to not even be able to walk up a flight of stairs without getting severely out of breath. Or trying to lift something heavy and not be able to move it an inch. This is a very real reality for people with poor circulation. They may not even know that this is why they are unable to perform specific physical tasks. They might think that they are unfit and that they need to try harder.

The reason why people with bad circulation are less physically capable is that their muscles are not getting enough blood and not at the correct speed. Their heart is also taking the strain as it is forced to pump harder to try to get the blood to where it needs to go.

 

16. Unexplained weight loss

These days everyone is obsessed with their weight. People go to the gyms on mass. They spend thousands on diet products and are always on the lookout for the next best thing that will make them thin without having to put in too much effort. This is the society we live in. The thinner, the better. It is for this reason, that if a person suddenly loses weight, they would not blink an eye.

They would more likely be thanking their lucky stars rather than sitting back and trying to figure out what is happening. But, and there is no argument here if a person loses weight and they can find no explanation for it they should not just brush it under the rug. This person needs to take a moment to try and figure out what has changed in their lifestyle. Are they more stressed than usual or possibly they have changed their diet? Maybe they have started a new medication?

Poor circulation does not necessarily affect one’s weight directly, and it is more of a secondary consequence. When a person’s blood is not getting appropriately circulated, they are more inclined to have a decreased appetite, after an extended enough period, this will lead to weight loss. There is also the case of malabsorption. If the digestive system is not getting the correct blood flow, then it does not operate properly. Food is not broken down and absorbed correctly, also leading to weight loss over time.

17. High blood pressure

Blood pressure is a bit of an odd physiological value. It is not one that is widely understood by those who do not work in the medical field. People know about their heart. They know that it beats and on the whole, they can find their pulse if they need to. Barring this, they can feel their heartbeat if they are paying enough attention. But heart rate is only one part of the equation. There is the speed at which the heart pumps blood around the body, and then there is the actual pressure that is created when the heart pumps this blood.

It is almost counter intuitive, but when the body is experiencing poor circulation, a person will have increased blood pressure. This is because the heart has to work harder to pump blood throughout the body. If blood is pooling in one’s limbs and muscles then when the heart tries to beat as normal, it is trying to fit more blood into a space that already has blood.

On the surface, a slight increase in blood pressure may not seem that dangerous. This is only somewhat true. The body is designed to handle periods of high blood pressure. The heart and blood vessels will not be immediately affected if blood pressure increases. There are times when it is normal to have elevated blood pressure. High blood pressure in the short-term will not necessarily cause long-term damage. But, if this high blood pressure persists for too long, then the body will start taking the strain. 18. A decrease in libido

Sex is very much a part of life. It is arguably one of the most critical aspects of the human experience. There are some who may even argue that it controls the world. This number is not one that can be tested, but it is safe to say that a lot of us, maybe even all of us, make a lot of our decisions with sex in mind. While we may have made quite a bit of progress in this arena over the last couple of years, we are not almost nearly where we need to be. We need to be able to talk about sex not only for our mental health but our physical health as well. There is also the appearance of little bags and dark rings underneath the eyes.

While these bags and rings may not be attractive, they are temporary. But, if you have dark rings underneath your eyes that won’t go away even though you are getting the correct amount of sleep. Then there may be other factors that should be considered. One of these being bad circulation. When it comes to dark rings underneath the eyes, these appear because blood is not moving the way it should. Instead, it is pooling in that area and not being circulated by the natural mechanisms in place. As mentioned, there are other causes for these dark rings other than poor circulation. But, there is a very easy way to check whether circulation is the culprit.

The way to do this is to press down on the rings while looking in the mirror. If the rings subtly clear up for a few seconds and then darken up again, then the problem is poor circulation. This little at home test should tell you whether you are having circulation issues, but it will not tell you the extent of the problem. A doctor can only do this after further testing.

20. Brittle nails

When one thinks of their health, they do not generally look to their nails as indicators. Many people do not even look after their nails, so they wouldn’t even actually know if something is wrong with them. The first thing that needs to be considered is the quality of the nail. Are the strong or are they brittle? Do they chip and break very easily or are they thick and hard to cut? If the nails are brittle, then there could be a circulation problem that exists. The reason for this is that nails, like every other part of the body, needs nutrients to grow.

If circulation is weak, then these areas will not be getting enough nutrients and they will, therefore, be weak and will not grow properly. Another way that nails can indicate the quality of circulation is by looking at the nail beds themselves. If you press down on them until they turn white and then release, look at how long it takes for them to go pink again. If this seems like it is taking too long, then there could be a prevailing circulatory problem.

When it comes too poor circulation, there are many symptoms which can be sought out simply because bad circulation will affect the entire body. By looking at one’s nails, you will not be able to make a full diagnosis, but it will give you a pretty good idea on the state of one’s blood circulation.

How to Improve Blood Circulation

• Add physical exercise to your daily routine.

• Stress can be one of the affecting causes of poor blood circulation. Practice meditation, mindfulness, and yoga to control your stress levels.

• Massage your body using warm olive oil and some drops of rosemary essential oil. Use it on your extremities such as hands, fingers, feet, and toes. As well as legs and arms.

• Try contrast hydrotherapy (jumping from hot to icy water and viceversa) to improve your blood circulation.

• Posture is a leading factor to blood circulation, either good or bad. Talk to a professional to make sure that you sit, walk, and stand properly.

• Caffeine is also not good for blood circulation as it causes anxiety. Change it for green tea or herbal teas in general.

• Alcohol intake must be reduced to a minimum.

• Smoking is bad either actively or passively, so we advise to quit smoking and to stay away from secondhand smoke.

• Add nuts, black pepper, cayenne pepper, seeds, onion, garlic, and ginger to your diet to boost your metabolism and blood circulation.

• Proper shoes are key to keep your legs and feet healthy and to help with blood circulation.

#Milhões de mortes cardiovasculares por baixo consumo de fruta e legumes

O consumo desadequado de fruta e produtos hortícolas poderá contribuir para milhões de mortes anuais globalmente por acidente vascular cerebral (AVC) e doenças cardíacas, demonstrou um estudo.

Segundo ainda o estudo liderado por Victoria Miller da Universidade Tuffs, EUA, foi estimado que uma em cada sete mortes cardiovasculares poderão ser atribuídas ao baixo consumo de fruta, e uma em cada 12 ao consumo insuficiente de vegetais.

A equipa de Victoria Miller definiu, com base em diretrizes nutricionais, o consumo ideal de fruta como sendo de 300 gramas diários, o que equivale a duas maçãs pequenas. O consumo ideal de produtos hortícolas é de 400 gramas diários, o que corresponde a três chávenas de cenouras cruas.

Os consumos médios nacionais de fruta e legumes foram recolhidos através de sondagens e dados sobre a alimentação representativos de 113 países, o que cobre cerca de 82% da população mundial.

A informação recolhida foi depois combinada com dados sobre as causas de morte em cada país e dados sobre o risco cardiovascular associado a um consumo desadequado de fruta e produtos hortícolas.

A equipa apurou que o consumo desadequado de fruta resultou em 1,8 milhões de mortes cardiovasculares só em 2010, enquanto o consumo insuficiente de produtos hortícolas causou um milhão de mortes. Foi apurado que globalmente, o consumo desadequado de fruta foi quase o dobro do de vegetais.

Os impactos foram mais pronunciados em países que apresentavam os consumos médios de fruta e legumes mais reduzidos.

“As prioridades globais de nutrição têm-se concentrado, tradicionalmente, em fornecer calorias suficientes, na suplementação de vitaminas e em reduzir os aditivos como sal e açúcar”, disse Dariush Mozaffarian, autor sénior do estudo.

“Estes achados indicam uma necessidade de expandir o enfoque para aumentar a disponibilidade e consumo de alimentos protetores como frutas, legumes e leguminosas – uma mensagem positiva com um enorme potencial de melhorar a saúde global”, recomendou.

Muitos pacientes com doenças virais, como o resfriado comum ou as conjuntivites virais, saem do consultório frustrados por não prescrevermos tratamentos efetivos contra estas infecções. Na verdade muitos queriam a receita de um antibiótico ou uma solução imediata. No entanto, vale ressaltar que essas doenças são virais e não podem ser tratadas com antibióticos.

São autolimitadas, duram em geral de sete a 10 dias e não há medicações que solucionem imediatamente essas infecções. O que precisamos lembrar constantemente é que na conjuntivite viral, por exemplo, o mais importante são as orientações para evitar o contágio e para o alívio sintomático.

As orientações fazem parte do tratamento e acompanhamento desses pacientes. Quais seriam elas?

  1. Lavar as mãos constantemente e não coçar os olhos;
  2. Não compartilhar toalhas, roupas de cama, etc;
  3. Desinfectar superfícies e itens compartilhados com álcool 70% para evitar transmissão;
  4. Orientar ao paciente que a conjuntivite viral é uma condição autolimitada, podendo haver piora do quadro nos primeiros quatro a sete dias. O quadro pode se arrastar caso o paciente tenha envolvimento corneano;
  5. Evitar contato com pacientes imunodeprimidos, crianças e gestantes;
  6. Restringir atividades para pacientes com exposição significativa a outros indivíduos durante o quadro (atestado);
  7. Compressas geladas aliviam os sintomas;
  8. O uso de lágrimas artificiais, preferencialmente sem conservantes, também pode aliviar os sintomas;
  9. Suspender o uso de lentes de contato até a resolução dos sintomas;
  10. Não existe benefício no uso de antibióticos ou corticoides na conjuntivite viral aguda. Os corticoides têm indicações específicas na fase tardia da doença;

#Formam-se novos neurónios até depois dos 90 anos de idade

A formação de novos neurónios, conhecida como neurogénese, ocorre até depois dos 90 anos e em cérebros com défice cognitivo e até com doença de Alzheimer, demonstrou um novo estudo.

Conduzido por uma equipa de investigadores liderada por Orly Lazarov, docente de anatomia e biologia celular na Faculdade de Medicina da Universidade de Chicago, EUA, o estudo teve como base a análise de tecidos de 18 cérebros de pessoas falecidas com idades entre os 79 e os 99 anos. A média de idades dos cérebros era de 90,6 anos.

A equipa procurou células estaminas neurais e neurónios recém-desenvolvidos nos tecidos. Como resultado, foram detetadas, em média, 2.000 células estaminas neurais em cada cérebro. Foram ainda identificados, em média, 150.000 neurónios em desenvolvimento em cada cérebro analisado.

A análise de um subgrupo dos neurónios em desenvolvimento revelou que o número de neurónios em desenvolvimento era significativamente inferior nos cérebros de pessoas com défice cognitivo e com doença de Alzheimer, em relação às que tinham uma função cognitiva normal.

Contudo, “o interessante é que observámos alguns neurónios novos nos cérebros de pessoas com a doença de Alzheimer e com incapacidade cognitiva”, disse Orly Lazarov.

A investigadora indicou ainda que as pessoas que tinham tido melhores pontuações em medições da função cognitiva possuíam uma maior quantidade de novos neurónios em desenvolvimento, em relação às que tinham tido piores pontuações, independentemente dos níveis de patologia cerebral.

Orly Lazarov considera que os níveis mais reduzidos de neurogénese no hipocampo estarão associados a sintomas de declínio cognitivo e de menor plasticidade sináptica, e não com o grau de patologia cerebral.

A ideia de continuarem a formar-se novos neurónios após a meia-idade, sem falar após a adolescência, é controversa pois estudos anteriores ofereceram resultados contraditórios.

A autora está muito entusiasmada com estes resultados pelas possibilidades terapêuticas que oferecem: “o facto de termos descoberto que estão presentes células neurais estaminais e neurónios novos no hipocampo de idosos significa que poderemos conseguir desacelerar ou prevenir o declínio cognitivo nos idosos, especialmente quando este começa, que é quando as intervenções podem ser mais eficazes”, concluiu.

Hoje é comemorado o Dia Mundial Sem Tabaco. É importante lembrar nesta data que é papel dos profissionais de saúde alertar todas as gestantes sobre os graves riscos que o tabagismo traz para a saúde da mãe e do bebê.

Um estudo publicado recentemente surge como um alerta importante para as gestantes fumantes. Recém-nascidos de mulheres que fumaram um ou mais cigarros por dia durante a gestação tiveram mais que o dobro do risco de morte súbita, em comparação com aqueles cujas mães não fumaram. E esse risco aumentou a cada cigarro, como mostra o estudo coordenado pela médica Tatiana M. Anderson do Center for Integrative Brain Research, do Seattle Children’s Research Institute, em Washington, nos Estados Unidos. O estudo foi publicado online em abril deste ano, no periódico Pediatrics.

Segundo os resultados, o índice de risco de morte súbita nos bebês caiu quando as mulheres reduziram a quantidade de cigarros ou pararam de fumar. Em comparação com as gestantes que não reduziram a quantidade de cigarros durante a gestação (mais da metade), os bebês das mães que o fizeram por volta do terceiro trimestre tiveram uma pequena queda (17%) do risco de morte súbita. E os bebês das gestantes que pararam de fumar no terceiro trimestre apresentaram uma redução ainda maior (23%) do risco.

Os pesquisadores analisaram dados estatísticos de 20.685.463 nascimentos e 19.127 casos de morte súbita, ajustando os critérios para raça e/ou etnia/origem de mãe e pai, idade da mãe e do pai, estado civil da mãe, escolaridade da mãe, ordem de nascidos vivos, número de consultas pré-natais, idade gestacional em semanas, tipo de parto, sexo do recém-nascido e peso ao nascer.

Vale destacar que a morte súbita inclui a síndrome da morte súbita do lactente, sufocação e estrangulamento acidentais no leito e causas mal definidas.

Comparado com recém-nascidos de mães não fumantes, o índice de risco de morte súbita e inesperada foi mais que o dobro para recém-nascidos de mães que relataram ter fumado durante a gestação.
O risco foi quase duas vezes maior com um cigarro fumado por dia, e aumentou linearmente até o patamar de 3,17 para as que fumaram um maço inteiro (20 cigarros) por dia.

“Essa correlação foi similar para cada trimestre quando modelada independentemente, mas o número médio de cigarros nos três trimestres juntos teve o maior poder preditivo”, explicaram os pesquisadores. Os resultados do estudo indicam que “os esforços de cessação do tabagismo podem ter um impacto maior na diminuição das taxas de morte súbita e inesperada quando direcionadas a pessoas que fumam menos de um maço por dia, em vez das que fumam mais de 20 cigarros por dia, que são os alvos mais tradicionais das campanhas”.

Mais da metade (55%) das mães que fumaram durante a gestação continuaram a fumar a mesma quantidade de cigarros que fumavam antes da gestação. Em média, aquelas que pararam de fumar no começo do terceiro trimestre (20%) reduziram o número de cigarros fumados durante toda a gestação em 58%, e as que reduziram o fumo no terceiro trimestre reduziram o número de cigarros em 33%. Esta foi uma “redução mensurável”, segundo os pesquisadores.

Em comparação com as gestantes que continuaram a fumar, o risco de morte súbita e inesperada foi ligeiramente menor no grupo que reduziu a quantidade de cigarros, mas a maior redução no risco foi observada no grupo que parou de fumar.

Na comparação com mulheres que não fumaram durante a gestação ou nos três meses anteriores, o risco de morte súbita e inesperada aumentou progressivamente nas mães que fumaram e pararam antes da gestação, naquelas que não fumavam antes, mas fumaram durante a gestação e nas que fumavam antes e durante a gestação.
Para aquelas que somente fumaram antes da gestação, a quantidade de cigarros não pareceu ter efeito sobre o risco de morte súbita e inesperada.

Os pesquisadores acreditam que as estimativas de tabagismo provavelmente são conservadoras porque não avaliaram a exposição ambiental ao fumo durante o pré-natal e o pós-parto, ou o tabagismo paterno, que é um fator conhecido para o risco de morte súbita e inesperada. Além disso, as estimativas de tabagismo dependem dos relatos das mulheres, que podem ter preferido não revelar o seu status de fumante.

“Estimamos que as taxas de morte súbita e inesperada nos Estados Unidos poderiam ser reduzidas em 22% se nenhuma mulher fumasse durante a gestação”, concluem os pesquisadores.

#Problemas de sono em adolescentes revertem rapidamente com limite de uso de ecrãs

O sono de adolescentes melhorou em apenas uma semana após limitar-se a exposição dos mesmos a ecrãs que emitiam luz azul, como telemóveis, tablets e computadores. 

Os achados são de um estudo conduzido por uma equipa de investigadores do Centro Médico da Universidade de Amsterdão, na Holanda.

Segundo os investigadores, o simples facto de os jovens limitarem o uso de ecrãs com luz azul à noite não só fez melhorar rapidamente a qualidade do sono dos mesmos, assim como reduziu sintomas de fadiga, falta de concentração e mau-humor.

Para o estudo, a equipa analisou os efeitos da exposição, pelos adolescentes, à luz azul em casa.

Foi apurado que os jovens que passavam mais de quatro horas diárias em frente a ecrãs começavam a dormir e despertavam, em média, 30 minutos mais tarde do que os que relatavam passar menos de uma hora por dia em frente a ecrãs. Mais, aqueles jovens apresentavam também mais sintomas de privação de sono.

Os investigadores conduziram um ensaio controlado e aleatório para avaliar os efeitos do bloqueio da luz azul com óculos e através da ausência de ecrãs, à noite, sobre os padrões de sono de 25 utilizadores frequentes.

Como resultado, os jovens participantes começaram a dormir e a despertar 20 minutos mais cedo, assim como relataram uma redução nos sintomas de privação de sono após apenas uma semana.

Estudos recentes revelaram que a exposição a demasiada luz à noite, especialmente luz azul emitida por ecrãs de smartphones, tablets e computadores pode afetar o relógio do sono, assim como a produção da melatonina, a hormona do sono.

La gastrite est l’inflammation de la muqueuse gastrique.

  • L’inflammation peut être provoquée par de nombreux facteurs, dont les infections, le stress, les lésions, certains médicaments et les troubles du système immunitaire.

  • Lorsque des symptômes apparaissent, il s’agit d’une douleur ou d’une sensation de gêne abdominale et parfois, de nausées ou de vomissements.

  • Les médecins fondent souvent le diagnostic sur les symptômes du patient, mais ont parfois besoin d’examiner l’estomac au moyen d’une sonde souple dotée d’une caméra (endoscopie haute).

  • Le patient est traité par des médicaments qui réduisent l’acide gastrique.

Si la muqueuse de l’estomac résiste à l’irritation et, en général, peut tolérer des acides puissants, dans la gastrite, elle est irritée et enflammée.

La gastrite est divisée en deux catégories d’après sa sévérité :

  • Érosive

  • Non érosive

La gastrite érosive est plus sévère que la gastrite non érosive. La gastrite érosive associe une inflammation et des abrasions (érosions) de la muqueuse de l’estomac. Les cellules qui produisent le mucus pour protéger la muqueuse de l’estomac de l’acide sont absentes ou endommagées. En général, la gastrite érosive se développe subitement (appelée gastrite érosive aiguë), mais elle peut aussi apparaître lentement (appelée gastrite érosive chronique), habituellement chez des personnes en bonne santé par ailleurs.

La gastrite non érosive est caractérisée par des modifications de la muqueuse de l’estomac qui vont de l’amincissement progressif (atrophie) de la muqueuse de l’estomac jusqu’à la transformation du tissu de l’estomac en un autre type de tissu intestinal (métaplasie). Souvent, plusieurs types de globules blancs s’accumulent dans l’estomac et provoquent différents degrés d’inflammation. Les globules blancs peuvent provoquer une inflammation de l’estomac entier ou uniquement de certaines parties.

Causes de la gastrite

Les types de gastrite spécifiques peuvent être provoqués par de nombreux facteurs, dont les infections, le stress, les traumatismes, certains médicaments et les troubles du système immunitaire.

La gastrite érosive est provoquée par l’alcool, le stress, les agents irritants comme les médicaments, principalement l’ aspirine et d’autres anti-inflammatoires non stéroïdiens (AINS), la maladie de Crohn, les infections bactériennes et virales, mais aussi par l’ingestion de produits corrosifs. Chez certains patients, la prise quotidienne d’une dose d’ aspirine pour bébés peut léser la muqueuse de l’estomac. Plus rarement, la gastrite érosive peut être provoquée par une exposition aux rayonnements, des infections virales (comme le cytomégalovirus) et des lésions directes (comme l’insertion d’une sonde nasogastrique).

La gastrite non érosive peut être provoquée par une infection à Helicobacter pylori.

Une gastrite infectieuse non provoquée par Helicobacter pylori est rare.

Une gastrite virale ou fongique peut se développer chez des personnes atteintes de maladies chroniques ou dont le système immunitaire est affaibli, comme en cas de sida ou de cancers, ou encore de traitement par des médicaments immunosuppresseurs.

La gastrite aiguë de stress, qui est une forme de gastrite érosive, résulte d’une maladie ou d’une lésion soudaine. Le site du traumatisme peut être différent de l’estomac. Par exemple, les brûlures cutanées étendues, les traumatismes crâniens et les blessures accompagnées d’hémorragies majeures sont des causes fréquentes. Le mécanisme par lequel une maladie grave provoque une gastrite n’est pas connu, mais il pourrait être lié à une diminution du flux sanguin dans l’estomac, une augmentation de la quantité d’acide dans l’estomac et/ou une incapacité de la muqueuse de l’estomac à se protéger et à se régénérer.

La gastrite radique peut être due à une irradiation de la partie inférieure de l’hémithorax gauche ou de la partie supérieure de l’abdomen, où les rayonnements peuvent irriter la muqueuse de l’estomac.

La gastrite post-gastrectomie est observée chez des patients qui ont subi une résection chirurgicale partielle de l’estomac (technique dite de gastrectomie partielle). En général, l’inflammation se produit au niveau des sutures chirurgicales. Il est possible que cette forme de gastrite soit la conséquence de la réduction chirurgicale de l’apport de sang à la muqueuse de l’estomac, ou de l’exposition de la muqueuse à une quantité anormale de bile (liquide digestif de couleur jaune verdâtre produit par le foie).

La gastrite atrophique provoque un amincissement important (atrophie) de la muqueuse de l’estomac et la disparition partielle ou complète des cellules qui sécrètent l’acide et les enzymes. Cette affection peut survenir lorsque des anticorps attaquent la muqueuse de l’estomac (on parle alors de gastrite atrophique métaplasique auto-immune). La gastrite atrophique peut également survenir chez certaines personnes chroniquement infectées par des bactéries Helicobacter pylori. Elle a également tendance à se produire chez les patients qui ont subi une gastrectomie partielle.

La gastrite à éosinophiles peut être due à une réaction allergique ou à une infestation par des vers ronds, mais la cause est en général inconnue. Dans cette gastrite, une importante infiltration d’éosinophiles (globules blancs) se produit dans la paroi gastrique.

La maladie de Ménétrier, une maladie rare dont la cause est inconnue, est un type de gastrite caractérisée par une paroi gastrique dont les plis s’hypertrophient et qui contient des kystes liquidiens. Cette maladie pourrait être due à une réaction immunologique anormale ou encore à l’infection par Helicobacter pylori.

La gastrite est en général asymptomatique. Lorsqu’ils sont présents, les symptômes varient selon la cause et peuvent associer une douleur, une sensation de gêne, des nausées ou des vomissements, troubles qui sont souvent dénommés indigestion (dyspepsie).

La gastrite érosive, la gastrite radique, la maladie de Ménétrier et la gastrite lymphocytaire peuvent provoquer des nausées ou des vomissements intermittents.

Une dyspepsie peut apparaître, surtout dans les gastrites érosives et radiques, ou dans les gastrites post-gastrectomie et atrophiques. On observe également une très légère dyspepsie dans les gastrites aiguës de stress.

Complications de la gastrite

La gastrite aiguë de stress peut s’accompagner de saignements dans les jours qui suivent une infection ou un traumatisme, tandis que les saignements ont tendance à se développer plus lentement en cas de gastrite érosive chronique ou radique. Si le saignement est léger et lent, il est possible de ne présenter aucun symptôme ou de remarquer seulement des selles noires (méléna) provenant de la couleur noire du sang digéré. Si le saignement est plus rapide, du sang peut être présent dans les vomissements ou dans les selles. Le saignement persistant peut provoquer des symptômes d’anémie : fatigue, faiblesse et sensation de vertige.

La gastrite peut évoluer en ulcères de l’estomac (ulcères gastriques), qui peuvent aggraver les symptômes. Si un ulcère perfore la paroi gastrique, le contenu gastrique peut se déverser dans la cavité abdominale, ce qui induit une inflammation et souvent, une infection de la muqueuse péritonéale (péritonite) et une aggravation soudaine de la douleur.

Certaines complications des gastrites se développent lentement. La fibrose cicatricielle et la sténose de la portion terminale de l’estomac, éventuelles conséquences d’une gastrite, surtout dans les formes radiques et à éosinophiles, peuvent entraîner des nausées intenses et des vomissements fréquents.

Dans la maladie de Ménétrier, une rétention hydrique et un gonflement des tissus (œdème) peuvent se produire en raison de la perte protéique à travers la muqueuse de l’estomac enflammée. Environ 10 % des patients atteints de la maladie de Ménétrier développent un cancer de l’estomac au bout de quelques années.

La gastrite post-gastrectomie et la gastrite atrophique peuvent s’accompagner de symptômes d’anémie, comme fatigue et faiblesse, en raison de la diminution de la production du facteur intrinsèque (une protéine qui se lie à la vitamine B12, ce qui permet à la vitamine B12 d’être absorbée et utilisée dans la production de globules rouges).

Un faible pourcentage de personnes atteintes de gastrite atrophique développent une métaplasie. Chez un pourcentage encore plus faible de personnes, la métaplasie évolue vers le cancer de l’estomac.

Diagnostic de la gastrite

  • Endoscopie par voie haute

Le médecin suspecte une gastrite lorsqu’une sensation de gêne est ressentie dans la partie supérieure de l’abdomen, accompagnée d’une douleur ou de nausées. En général, des examens complémentaires ne sont pas nécessaires. Cependant, si le médecin est incertain du diagnostic ou si des symptômes ne disparaissent pas avec le traitement, une endoscopie haute peut être réalisée. Pendant l’endoscopie haute, un endoscope est utilisé (une sonde souple dotée d’une caméra) afin d’examiner l’estomac et une partie de l’intestin grêle. Si besoin, une biopsie de la muqueuse de l’estomac (ablation d’un échantillon de tissu pour l’examiner au microscope) peut être réalisée.

Traitement de la gastrite

  • Médicaments qui réduisent la production d’acide et des antiacides

  • Parfois, les antibiotiques traitant l’infection à H. pylori

  • Traitements pour arrêter le saignement

Indépendamment de la cause de la gastrite, les symptômes peuvent être soulagés en prenant des médicaments qui neutralisent ou réduisent la production d’acide gastrique et en arrêtant la prise des médicaments à l’origine des symptômes.

Médicaments pour le traitement de la gastrite

Pour les symptômes légers, la prise d’antiacides, qui neutralisent l’acide déjà produit et libéré dans l’estomac, suffit généralement. De nombreux antiacides peuvent être achetés sans prescription médicale et sont disponibles sous forme de comprimés ou de liquide. Les antiacides incluent l’hydroxyde d’aluminium (qui peut provoquer une constipation), l’hydroxyde de magnésium (pouvant provoquer une diarrhée) et le carbonate de calcium. Parce que les antiacides peuvent interférer avec l’absorption de nombreux médicaments, il faut consulter un pharmacien avant de prendre des antiacides en cas de prise concomitante d’autres médicaments.

Parmi les médicaments qui diminuent la production d’acide figurent les inhibiteurs du récepteur H2 de l’histamine (anti-H2) et les inhibiteurs de la pompe à protons. Les anti-H2, en général, sont plus efficaces que les antiacides pour diminuer la symptomatologie, et de nombreux patients les trouvent beaucoup plus pratiques. Les médecins prescrivent le plus souvent des inhibiteurs de la pompe à protons pour la gastrite associée à des saignements.

Les médecins peuvent prescrire du sucralfate, qui aide à recouvrir la muqueuse de l’estomac et à la cicatriser, et prévient également les irritations.

Lorsque la gastrite est due à une infection par H. pylori, des antibiotiques sont également prescrits.

Gastrite érosive

Les personnes qui présentent une gastrite érosive doivent éviter de prendre des médicaments qui irritent la muqueuse de l’estomac (comme les AINS). Certains médecins prescrivent les inhibiteurs de la pompe à protons ou le misoprostol pour favoriser la protection de la muqueuse gastrique.

Gastrite aiguë de stress

La plupart des personnes qui souffrent de gastrite aiguë de stress guérissent complètement lorsque la maladie sous-jacente, la lésion ou l’hémorragie est contrôlée. Cependant, la gastrite aiguë de stress provoque chez 2 % des patients en unité de soins intensifs d’importants saignements, qui peuvent être mortels. Les médecins essayent donc de prévenir la gastrite aiguë de stress due à des maladies, à des traumatismes ou à des brûlures graves. Des traitements qui diminuent la sécrétion d’acide gastrique sont très souvent administrés après une chirurgie et dans la plupart des services de soins intensifs pour prévenir la gastrite aiguë de stress. Ces médicaments sont aussi utilisés pour traiter les ulcères en formation.

En cas d’important saignement dû à une gastrite aiguë de stress, une vaste gamme de traitements a été proposée. Seuls quelques-uns d’entre eux améliorent véritablement le pronostic. Les vaisseaux qui saignent peuvent être temporairement refermés par la chaleur (cautérisés) pendant une endoscopie. Cependant, le saignement reprend souvent si la maladie sous-jacente persiste. En cas d’hémorragie, l’ablation totale de l’estomac peut s’avérer nécessaire pour sauver la vie du patient.

Autres types de gastrite

Il n’existe pas de traitement curatif de la gastrite post-gastrectomie ou de la gastrite atrophique. Les sujets qui présentent une anémie liée à une diminution de l’absorption de vitamine B12, qui se manifeste dans les gastrites atrophiques, devront recevoir une injection de la vitamine tout au long de leur vie.

Les corticostéroïdes ou la chirurgie peuvent être nécessaires pour libérer l’obstruction, provoquée par la gastrite éosinophile, de l’ouverture distale de l’estomac.

L’ablation de tout ou partie de l’estomac peut guérir la maladie de Ménétrier, pour laquelle aucun traitement médicamenteux n’est efficace.
